Home visits

To request a home visit, please submit the request online or by phone and ask for a home visit as soon as possible after 8:00 AM between Monday to Friday (excluding Bank Holidays). If possible please try to contact teh practice before 10:00 if you require a home visit.

A telephone appointment will be offered with a clinician to assess the condition over the phone and plan a home visit if required. The clinician will consider if your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance.

Home visits are only available for patients who are housebound because of illness or disability and reside within the practice boundary.

Please note that several patients can be seen in the practice during the time that it takes to make one home visit; there are also better facilities for examining and treating patients at the health centre.
