Appointments (Urgent/Routine)
For life-threatening conditions, please attend your local hospital’s A&E department immediately. Booking an appointment at the surgery may jeopardise your health and delay essential treatment.
We are currently operating an online triage system.
Please complete the request form below with accurate details about your health concern.
We aim to offer you an appointment on the same day, based on clinical need.
This means you may not see your preferred doctor, but you will be seen by a qualified and experienced clinician who can help with your problem.
Please use this service appropriately.
Misusing urgent appointments can delay care for patients who are seriously unwell.
Requests for sick notes, repeat prescriptions, or referrals are not suitable for urgent appointments.
These should be made through the appropriate non-urgent channels.
To request an appointment (non-emergencies) with a clinician please complete a short request form at 8:00 AM from Monday to Friday (excluding Bank Holidays) on the link below.
- If you need assistance beacuse you are not able to read English or you do not have access to the internet:
- You can call the practice on 01612573338 and we can help you to complete the request form.
- You can visit the surgery to speak with a member of our team during opening hours.
When you get in touch, our reception staff may ask you for details of your problem. The doctors ask them to do this so they can prioritise same day appointments.
All our staff must keep anything you tell them confidential. We will use the information you give us to choose the most suitable doctor, nurse or health professional to help you.

Home visits
To request a home visit, please submit the request online or by phone and ask for a home visit as soon as possible after 8:00 AM between Monday to Friday (excluding Bank Holidays). If possible please try to contact the practice before 10:00 if you require a home visit.
A telephone appointment will be offered with a clinician to assess the condition over the phone and plan a home visit if required. The clinician will consider if your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance.
Home visits are only available for patients who are housebound because of illness or disability and reside within the practice boundary.
Please note that several patients can be seen in the practice during the time that it takes to make one home visit; there are also better facilities for examining and treating patients at the health centre.
